NARRATIVE DESCRIPTION

Description of the Incident: Pet arrived Houston for overnight kenneling as a rest stop after 16 ½ hour flight from Guam...

Description of the Cause of the Incident: The combination Jack Russell Terrier and Chihuahua was being walked with a leash to relieve herself, got spooked, and darted out in to the road where she was hit by a car and killed at the hands of the vendor employee. Tragic accident.

Description of any corrective action taken: Have implemented a two leash policy where one leash is held in the hand of the walker and the other goes around the waist of the walker, the other end of each attaching to the collar of the dog being walked. In 10 years no dog has ever escaped this way. Did not escape from a kennel but at the hands of the vendor employee.
