INCIDENT #1: UA FLIGHT #724 CAT: MALE Honolulu (HNL) to San Francisco (SFO) to Baltimore (BWI) NAME: BOGIE December 22, 2013 Owner/Guardian REDACTED NARRATIVE DESCRIPTION Description of the Incident: Animal escaped Description of the Cause of the Incident: HNL agent was drilling required holes in crate to secure the crate door with the plastic zip ties when the door opened and the cat escape Description of any corrective action taken: HNL Cargo Manager has designated two rooms as “Pet Rooms” in the warehouse where all physical searches and securing of kennels will occur. In this closed room, the animal can be released from its crate with pet owner present while kennel is physically searched and required holes are drilled into kennel by carrier personnel.
