Incident 3 Carrier: Alaska Airlines Flight: 575 (San Diego, CA to Portland, OR) Date of incident: Friday, June 6, 2014 Time of incident: Approximately 4:51 PM PT Description of animal (including name): A dog named Bear, unknown age, sex or breed. Narrative description of incident: Upon arrival in Portland, OR our ramp agents noticed, that a dog named Bear had blood in its kennel. The dog appeared to have been pawing to escape the kennel and pulled out a toe nail, sometime during the flight. DOT Animal Incident Report – June 14 – Redacted Narrative description of the cause of the incident: A dog named Bear arrived with blood in its kennel and a missing toe nail. There is no evidence to suggest that the airline’s handling contributed to the animal’s injuries. Narrative description of any corrective action taken in response to the incident:
