DOT PET INCIDENT REPORT Carrier and Flight Number American Airlines Flight #1418 Date and Time of the Incident 21 May 2015 (G) 22:34 Description of the Animal, Including Name Dog, Beagle mix, Name: Dash Narrative Description of Incident During transportation from airport terminal to cargo facility the Dog escaped. The young beagle mix, Dash, was found to be in good health with the exception of a fat lip and a scrap on his inner thigh. The cause of damage to the kennel is undetermined. The dog was transported to a Veterinarian facility for the initial examination. Narrative Description of Cause of the Incident Unable to determine cause of injury. Narrative Description of any Corrective Action Taken in Response to Incident Station proceed to have employee re-take Live Animal Handling training
