Description of Incident: Upon arrival of HA202 on June 27. 2018, our ramp agent found the dog unresponsive. Chief Agent, Albert Nekoba, was notified and met owner to advise him that his dog had passed. Manager on Duty, Luana Gibson, also spoke to owner.
Cause of Incident: unknown
Corrective Action Taken: Consumer Affairs refunded the pet fees back to the original form of payment.
