Carrier: United Airlines Flight Number: 252 Date & Time of Incident: 06/22/2016 Type of Incident: Injury Description of Animal: Breed: Rhodesian Ridgeback Animal Name: PRINCESS Description of Incident: Upon arrival at Houston, TX (IAH), Princess was discovered loose within the cargo hold of the aircraft. She was immediately and safely recovered and returned to her shipping container. Cause of Incident: The process of escaping the container resulted in a minor self-inflicted injury to one toe nail. Corrective Action Taken: Application of releasable kennel ties may have contributed to the animal’s ability to escape from its shipping container. HNL will review United’s releasable cable tie procedure with cargo agents to prevent future recurrence
