Incident 1 Carrier: Alaska Airlines Flight: 857 (Seattle, WA to Maui, HI) Date of incident: November 30, 2011 Time of incident: Approximately 12:40 P.M.

Description of animal (including name): An 11-year-old, male, Yorkshire terrier dog.

Narrative description of incident: A Maui veterinarian met the passengers upon arrival in Maui to examine and clear their dog and cat. The dog was found deceased in his kennel. The passenger explained that he was an older dog with congenital tracheal malformation and was in failing health. The veterinarian cleared the passenger’s cat and they were also able to take the dog’s remains with them to their destination.

Narrative description of the cause of the incident: A dog with a pre-existing medical condition was found dead upon entry inspection into Maui. There is no evidence to suggest that the airline’s handling contributed to the animal’s death.

Narrative description of any corrective action taken in response to the incident: No action required.
