Report Details
(1) Carrier and flight number; UA200 (2) Date and time of the incident; February 4th, 2025 at 1852 (3) Description of the animal, including name, if known; Name: Ina Breed: Catahoula Leopard (4) Name and contact information of the owner(s), guardian, and/or shipper of the animal; (5) Narrative description of the incident; The dog was accepted for flight UA200 on February 5th. A routine inspection confirmed the dog appeared in good health, and the health certificate was in order. The dog was transported planeside and loaded onto the aircraft, with the ramp agent’s visual check again confirming the dog appeared healthy. Upon arrival in HNL on February 4th, the unloading ramp agent found the dog motionless and unresponsive. All indications suggest the dog died during the flight. (6) Narrative description of the cause of the incident; Pre-existing condition (7) Narrative description of any corrective action taken in response to the incident; and None: There were underlying health issues with the dog that were not detectable at the time of acceptance or noted on the health certificate. We have found no actions we could have taken to prevent this. (8) Name, title, address, and telephone number of the individual filing the report on behalf of the air carrier.
